Welcome to the referrer page for healthcare professionals. You can refer your patients to the NKI for:
- (rapid) diagnostics
 - second opinion
 - Takeover of treatment
 
Referrals from medical specialists go through the registration form below.
Referring your patient
For a quick and efficient referral, we ask you as a referrer to complete the referral form completely and to send all relevant documentation, so that we can schedule the patient at the right outpatient clinic. If you add additional relevant information (such as information from the pulmonologist or cardiologist), we can schedule the patient for surgery more quickly.
After this form has been completed and submitted, the planning department processes the form. The patient is invited to the outpatient clinic for this problem. Our outpatient clinics are largely organ-specific and multidisciplinary. After the visit to the outpatient clinic, you will be notified as soon as possible.
Referral from abroad
Would you like to refer a patient from outside the Netherlands to the Netherlands Cancer Institute? You can use the e-mail address tpp@nki.nl for this. Please include the following patient details in your referral:
- Initials
 - Gender
 - Date of birth
 - Address, postal code, city, country
 - Telephone
 - Enamel
 - Has the patient been to AVL before?
 - Diagnosis/suspicion
 
Referral for Radiotherapy
Do you have a referral for the Radiotherapy Department? Then we ask you to send an e-mail to: RT-medsec@nki.nl and to send the following information:
- a referral letter containing the relevant medical history
 - copies of relevant pathology reports
 - copies of relevant operation reports
 - copies of relevant X-ray reports
 
Radiology images and reports can be exchanged digitally via the Twiin image exchange portal.
If it is urgent, you can call 020 512 9111. The medical data can then be given to the patient.

Referral for treatment with Lutetium-177-PSMA
Would you like to refer a patient for Lutetium-177-PSMA therapy?
Then we would like to receive the following information via a digital referral to our uro-oncologists:
- Referral letter with relevant medical history
 - Most recent lab values, max. 2 months old
 - Scan and report most recent PSMA PET/CT.
 - Scan and report most recent diagnostic CT thorax/abdomen.
If necessary, we repeat the examinations. 
NB: severe urinary incontinence is a contraindication for treatment in our department. It is also important that your patient is self-reliant during the day admission.
After referral, your patient will be discussed at our therapy meeting (Tuesday 11.30 am). As a referrer(s) you are welcome to join online. It is also possible to join if you would like to consult with our team prior to a referral. Please register in advance via ngtherapie@nki.nl.
Determining circulating tumor DNA
The Netherlands Cancer Institute offers referrers the opportunity to request a circulating tumor DNA (ctDNA) analysis. To do this, go to our laboratory forDNA diagnostics.

Phase 1 study
If there are no more regular treatment options for your patient, there may still be experimental treatment options in the context of the so-called phase 1 studies. If you want to know if your patient may be eligible for one of our phase 1 studies, you can send a short email to fase1@nki.nl.
If you provide at least the following information in the email, we can assess whether your patient could possibly participate in an experimental treatment in our hospital and whether referral for this is useful. We aim to answer your email within a few days.
Minimal data in mail:
- Referrer Name
 - Referrer's phone number
 - Patient Name
 - Patient's date of birth
 - Tumor type
 - Previous treatments
 - Known results of molecular diagnostics
 
NB: Are you a patient and do you want to know if you are eligible to participate in a phase 1 study? Then ask your treating specialist to contact us.
